Well I started it but then the English started to go downhill on the second major page and the questions got repetitive. I'm extremely surprised that you supervisor didn't make you do more QA on this before unleashing it on the public because in its current state I don't believe you are going to get much of a response rate and the results will not be reliable enough to be a good basis for your dissertation.
Sorry. Perhaps you could revise and test it then try us again.
